CERTIFICATIONAND AUTHORIZATION TO OBTAIN AND RELEASE CREDIT INFORMATION: By signing below, each owner, officer or guarantor (collectively, the “Applicant” or “Applicants”) individually and as the duly authorized representative of the Applicant certifies, understands and agrees that: (a) this Credit Application (“Application”) is made for the sole purpose of obtaining commercial credit and is not a consumer transaction; (b) by completing this Application in any capacity, including, without limitation, as the principal of a formal business entity or as a sole proprietor or individual, even if the signatory’s personal creditworthiness alone is the basis for establishing eligibility for credit or services sought by this Application, the Applicant is seeking credit for commercial, business or agricultural purposes only, and not for personal, family or household use; (c)that the information contained in this Application is true, correct and complete; (d) that this information is being furnished to Putnam LeasingCompany I, LLC (“PLC”) for the purpose of inducing PLC or its successors, assigns, transferees or one or more third party funding sources (“FundingSources”) to extend credit to Applicant, and that PLC or its Funding Sources intends to rely upon such information; and (e) that PLC or its Funding Sources will retain this Application whether or not it is approved. In connection with PLC’s evaluation of and credit inquiry with respect to this Application, theApplicant hereby authorizes PLC its Funding Sources and any dealer, distributor or manufacturer of the vehicle(s) to be financed or leased, to the extent PLC deems necessary or appropriate, from time to time to: (1) obtain, verify and release information about the Applicant, (2) obtain personal and corporate credit reports and information on the Applicant from credit reporting agencies, (3)contact and inquire of any person or entity of any kind regarding the financial information of or references of the Applicant, including bank and trade references and information furnished by credit reporting agencies, and (4)otherwise investigate the credit of the Applicant. The Applicant hereby instructs all credit reporting agencies to provide PLC and its Funding Sources with such credit reports upon request. The Applicant authorizes all trade references and bank references named in this Application to release and furnish information to PLC and its Funding Sources concerning the Applicant’s credit, purchase, and payment history. The Applicant authorizes PLC and its FundingSources to release information to other creditors or credit reporting agencies regarding their credit experience with the Applicant. In the event thisApplication is approved, this is a continuing authorization for all present and future disclosures of financial information, account information and credit experience on the Applicant for purposes of updating, renewing, extending, reviewing, servicing, facilitating, and collecting on any outstanding indebtedness under all loans, leases or extensions of credit, as well as other legitimate purposes associated with this Application and the Applicant. ThisApplication does not obligate PLC or its Funding Sources to extend credit to theApplicant or any entity(ies) or person(s) identified in this Application and is not and shall not be construed to be an offer of credit or an agreement to sell, finance or lease to the Applicant. A decision to grant or deny this Application will be made by PLC and its Funding Sources in their sole and absolute discretion. PLC and its Funding Sources shall have no obligations under thisApplication unless and until this Application is approved. The individual signing this Application affirmatively states, represents and warrants that he or she is duly authorized to do so on Applicant’s behalf.
